What do I do with 3 years old with 39.5 fever after hours. I have no paracetamol at home or any other medication.

If the child still drinking liquids and responding in a normal way then 39,5 can be a normal fever. Make your child comfortable by placing a cold washcloth on their forehead. If the child becomes unresponsive call 112.
